The dress (horrible and tragic! who the hell puts different fabric shoulders on a dress? That didn’t even look good in the middle ages. for peasants.) looks like it is tight in the top (as you don’t see any side-flair like a woman with curves like that would actually have).
This would produce the effect of the picture.
If they were fake, you’d see them poking to the side, producing more shadow (regardless of how tight the top was), or being more round.
My guess would be she’s pretty flat-chested and making the most of what she has.

As a member (and arguably the president) of the itty bitty titty committee, I have to say that her boobies are real thanks to some combination of tape, a push up bra, and those little chicken cutlet things you can buy from any department store/Victoria’s Secret. Seriously those chicken cutlet things are Heaven-sent.
