Start small (e.g. your index finger) and work up to larger sizes. Plenty lubrication is key to ease penetration (past the sphincter muscle) and not to rip the very delicate tissue inside. Once you get a good sized plug “locked in” (when the sphincter muscle grips around the waist of the plug) the feeling is incredible, with or without vibration. Move around, walk, move your hips, and yes, definitely leave it in for your orgasm, while your dick is in someone else …
However, I find getting it out more tricky than getting it in. Both ways, “pushing” a little, like shitting, helps getting the widest part past the sphincter muscle.

For question #2: yes, there are plugs out there that have a little vent hole and are comfortable to wear for several hours.
